Advising Forms

PSY Override Form

Overrides are exceptions to standard enrollment policies and are not guaranteed. Common reasons students request overrides include:

• A course needed to graduate on time is full.
• Missing or in-progress prerequisites.
• Degree timing concerns.

To request an override:

• For PSY courses: use the Psychology Override Form.
• For courses in other departments: reach out to the department offering the course.

Independent Study Form

Students participating in undergraduate research or serving as a PSY undergraduate assistant may be eligible to earn independent study credit through PSY 490 or PSY 491. Students should first connect with a faculty member or supervising instructor to discuss the experience, expectations, and appropriate course credit option.

The Independent Study Form requires information developed in consultation with the instructor, including:

• Course code and semester information.
• Project description and learning outcomes.
• Contact hours and number of credits requested.
• Relevant preparation or experience.
• Description of completed work and evaluation methods.

Declare my minor

If you plan to add a College of Social Science Interdisciplinary Minor, you can request the minor be added to your account via the CSS Interdisciplinary Minor Form. If you select an “Approval Required” minor, the form will provide prompts to ensure you meet the necessary requirements. 

For all other minors, please directly contact the department offering the minor to request the minor be added to your degree plan.

FERPA Authorization for Release Forms 

FERPA protects the privacy of student educational records. These forms may be required if you would like Psychology advisors or faculty to:

• Provide a recommendation or reference.
• Share academic information with another individual.
• Speak with parents or supporters about your academic record.
• Include additional participants in advising meetings.
